This questionnaire is a self-assessment tool that focuses on attitudes, values and abilities related to starting a new church in the city. After reading each item, please indicate the degree to which you feel that statement is true of you. Please read each choice carefully and circle the number of the response which best expresses your feeling.

80-100: Potential Church Planter: you match many of the qualities of effective church planters. Seek ways to confirm your calling and placement.

60-79: Church Planter Associate: Continue checking it out, you could be an excellent team member with a church planter.

40-59: Learner: Talk to a leader in your life and explore special ministries related to church planting which will give you added experience.

15-39: Investigator: Your gifts will blossom in other areas of ministry.
